PATIENTS RIGHTS AND RESPONSIBILITIES

appointmentsOur purpose is to provide good quality practice to all patients.  You can help us to provide this care by attending your appointment on time.  We would ask you to take care not to introduce other patients into the consultation if they don’t have an appointment as this can create an unnecessary delay for other patients.

Administrative staff form an important part of our team.  They help you and us by organising clinics, issuing prescriptions, taking messages etc.    They are working for you and deserve your courtesy and respect.  If you are are upset about something, bring it to the attention of one of the Doctors, because they ultimately have the responsibility when things go wrong.

Stream Street Surgery is a training practice.  We feel privileged to be training the Doctors of the future, however we cannot do it effectively without your help.  Occasionally you may be asked to participate in video surgeries, although you are welcome to decline if this would make you feel uncomfortable.

Under the Freedom of Information Act you have the right to access your records.  Contact Colette to access this service.
